I use it on two medium-to-high loaded mail servers (~50,000 incoming 
emails/day) for about a month without any problems.

However about 90% of all connections are blocked prematurely by another 
little program (in combination with rblsmtpd) I hacked in an hour or 
two by modifying http://www.zentus.com/rhost-check.html, that sets 
environment variable RBLSMTPD on "bad" remote host names (empty or 
sthg. containing dsl, dial, dyn, ppp, pool etc).

The rest goes through recipient check (cvm-vmailmgr) and finally through 
the policyd checks.

I recognized a slight increase of spam in the recent month (~8/day to my 
personal addresses, but most comes over ezmlm driven mailing lists) and 
two joe-jobs to other addresses of those servers, so I'm considering 
writing a SPAM plugin for mailfront one day... I'm still waiting for 
Bruce to improve the plugin chain logic. Seems he's busy with other 
things.
